A drug manufacturer must get approval for how much of a drug it plans to produce, as well as the timeframe. If a shortage develops (because, say, the FDA shuts down a competitor’s plant), a drug manufacturer cannot increase its output of that drug without another round of approvals. Nor can it alter its timetable production (producing a shortage drug earlier than planned) without FDA approval.

Montreal, which has about three million people, has three MRI devices, while Allegany County, Maryland, where I work, has 80,000 people and three such devices. In Montreal, a person in need of an MRI has to wait six months (unless the person has political connections), while in Allegany County, the wait is miniscule, perhaps a day to set up the appointment.

Every media outlet in the country is echoing the Democrat talking points, crowing about what a tremendous defeat this was for the Republican health care plans. I'm not ready to accept that, but if it's true, it would explain why Republicans are running from the 2012 campaign as fast as their feet can carry them.

I like this column because it points out just how conservative Ryan's plan is. He's postponing the hard cuts for a decade and using figures cooked up by Alice Rivlin for god's sake. But there's no logic allowed in this debate. In 5 years or so the Democrats will put forth a plan that is almost identical to Ryan's proposal, and they will be praised as the saviors of Medicare.
